I've found this will happen if you install the plugin when you have SEF set to on.

To fix it we go to the PHP Admin on the server, find the jos_plugin, set the Joomla SEF to zero for off.

Then we'll uninstall the plugin. Reinstall it. Turn the SEF back on and we're in good shape.
