Also, now the URLs of the subcategories are like this: subdomain.domain.com/top-category-name/article-name. The /top-category-name/ should not be there.

Check your setting in Content JoomSEF extension parameters, namely the "Add categories to URL" on the "Extension" tab and "Add Menu Title to URL" on the "Common" tab.

It seems that there's a bug in our current Content extension when "Add categories to URL" is set to "No". If it's OK for you to have only the sub-category present in URL, you can switch "Add categories to URL" to "Only last one".
We'll fix the bug in our next version of JoomSEF.
