Ideally, I don't need home.

I notice that you have to purge the cache within Artio and then also within Joomla.

I've given up. Seems to be causing more problems than solutions.

All I need is my URL to look like this:

nowcosmetic.co.uk/botox-information
nowcosmetic.co.uk/dermal-fillers
nowcosmetic.co.uk/article-about-something

Please follow these steps:
1. open Manage Extensions page in JoomSEF
2. open the parameters for extension the URLs are created by (if they are standard Joomla articles, open the Content extension parameters)
3. on the Extension tab set "Add categories to URL" to "No"
4. on the Common tab set "Add menu title to URL" to "No"
5. save changes
6. open Manage SEF URLs page in JoomSEF
7. delete old SEF URLs
8. on JoomSEF's Control Panel on the Maintenance tab click the Clear Cache button
9. clear any other caches you use - the cache in Joomla and caches of any 3rd party components
10. open the homepage of your website - the SEF URLs should be recreated correctly
